Around the world, cats, dogs, and other pets are an integral part of many families. But while improvements to diet and veterinary care have brought increased lifespans, they have also seen an increase in health conditions that affect elderly pets. Shimada Akio is Japan's first veterinary prosthetist, pioneering custom prosthetics and rehabilitation schemes that preserve quality of life for animals who would once have been beyond help, including pets, wild animals, and zoo residents.

"RISING"
-
0m 08s
Shachi the chihuahua is ten years old.
-
0m 11s
Equivalent to her mid-50s in small dog years,
-
0m 14s
she is approaching senior status.
-
0m 19s
Due to the impact of heart disease and poor circulation,
-
0m 23s
she has recently had surgery
-
0m 24s
to amputate her right hind leg at the ankle,
-
0m 27s
compromising her mobility.
-
0m 31s
Her owner is concerned.
-
0m 33s
Our house has slate-tiled floors,
-
0m 36s
and the stump clacks against them.
-
0m 38s
It looks painful.
-
0m 42s
She wonders how the disability will affect Shachi in old age.
-
0m 48s
As she gets older, it might be hard to poop with just three legs.
-
0m 56s
Her regular veterinarian sees no simple surgical solution.
-
1m 06s
So, they turn instead to Japan's leading veterinary prosthetist,
-
1m 10s
Shimada Akio.
-
1m 16s
I'm thinking the prosthetic should fit up to the top of her leg.
-
1m 23s
First, we'll take a plaster cast.
-
1m 28s
To get an exact fit, Shimada makes a plaster mold of Shachi's stump.
-
1m 38s
And this will be used to form the basis of a prosthetic limb
-
1m 42s
that will restore to her the gift of mobility.
-
1m 46s
Elderly dogs suffer from a wide range of physical difficulties.
-
1m 51s
Take Yorkshire terrier, Lien.
-
1m 56s
Now 12 years old, she was raised from a puppy by owner Maruo Yukiko.
-
2m 05s
She was such a good walker, but now it seems tough for her.
-
2m 11s
Lien's left hind leg has become prone to painful dislocations,
-
2m 16s
hampering her mobility.
-
2m 19s
She's 12 years old now, but she loves her walks.
-
2m 23s
I hope there's a way to keep her walking as long as possible
-
2m 27s
without resorting to surgery.
-
2m 33s
It looks sore.
-
2m 36s
Her leg's twisted inwards, and she can't extend it herself,
-
2m 40s
so we'll fit a brace to do that for her.
-
2m 46s
Shimada fields numerous requests like these
-
2m 49s
from the owners of elderly and disabled pets
-
2m 52s
struggling with their mobility.
-
3m 00s
If I'm going to make something, I want to do it properly.
-
3m 05s
Restoring happiness to pets and their owners is very fulfilling for me,
-
3m 10s
so that's what I want to keep on doing.
-
3m 16s
This time on "RISING,"
-
3m 18s
we follow a prosthetics pioneer
-
3m 20s
creating a new lease of life for infirm animals.
-
3m 24s
"A New Lease of Life for Infirm Animals" Veterinary Prosthetics Pioneer - Shimada Akio
-
3m 31s
The west-Tokyo suburb of Machida is home to
-
3m 34s
Shimada Akio's prosthetics workshop.
-
3m 42s
Here, with the assistance of four staff,
-
3m 45s
Shimada creates his veterinary prosthetics and orthotics.
-
3m 50s
Today, he's working on a leg for Shachi,
-
3m 53s
the chihuahua who lost her foot due to cardiovascular issues.
-
3m 59s
The plaster mold he made was used to cast this model of Shachi's leg.
-
4m 06s
Every item is made to measure for individual animals.
-
4m 10s
The prosthetic limbs are covered in the same material
-
4m 13s
that wetsuits are made from.
-
4m 15s
And the smaller the patient, the more precise the work.
-
4m 22s
A ten-kilo dog is five times bigger than a two-kilo dog.
-
4m 27s
So of course, you have to adjust the size and the materials to match.
-
4m 33s
That's a given.
-
4m 37s
The socket will be lined with shockproof gel.
-
4m 40s
Sold to protect furniture and ornaments from earthquakes,
-
4m 44s
this material provides the ideal shock absorption for little legs.
-
4m 50s
The cover is stitched by Shimada's colleague, Kaneda Yumi.
-
4m 58s
This, too, calls for some non-standard approaches,
-
5m 02s
like bringing the inner seam outside the garment.
-
5m 08s
For elegant clothes, you'd have the seam inside.
-
5m 12s
But that can irritate dogs' skin, so we bring it outside.
-
5m 19s
Meanwhile, load-bearing segments are stitched by hand
-
5m 22s
rather than machine.
-
5m 27s
Hand stitching affords a little bit more give.
-
5m 31s
The stiffness of machine stitching
-
5m 33s
can sometimes be painfully constrictive.
-
5m 40s
The "foot" of the prosthetic is made from the same stiff rubber
-
5m 44s
as shoe soles, machined to the nearest millimeter.
-
5m 52s
And once all the parts are assembled,
-
5m 54s
Shachi the chihuahua's new hind leg is complete.
-
6m 02s
With something this small,
-
6m 04s
even a minor size discrepancy can hamper contact with the ground.
-
6m 09s
It's deceptively difficult, so we have to take appropriate care.
-
6m 18s
Two days later,
-
6m 19s
Shimada visits Shachi's regular clinic for a trial fitting.
-
6m 27s
Even if the fit is perfect,
-
6m 29s
it's not uncommon for dogs to reject their new limbs.
-
6m 36s
Let's go for a walk outside.
-
6m 46s
She's walking freely!
-
6m 50s
And so naturally, too.
-
6m 57s
Shachi has taken to her new leg like a duck to water.
-
7m 03s
She seemed happy to be able to walk again as usual.
-
7m 08s
She wasn't fazed at all.
-
7m 12s
Shachi can't speak but she looked pretty happy.
-
7m 17s
Meanwhile, for Lien, the dislocation-prone Yorkshire terrier,
-
7m 21s
Shimada has created a custom orthotic brace.
-
7m 34s
You can do it!
-
7m 38s
Wow, just look at her go.
-
7m 43s
Amazing!
-
7m 45s
Just like she's young again.
-
7m 48s
Maybe this can keep her mobile 'til the end!
-
7m 53s
For owner Maruo, it's a major relief to see her hopes
-
7m 56s
for Lien's lifelong mobility restored.
-
8m 04s
Higashi Sumiyoshi, Osaka.
-
8m 06s
Shimada doesn't just work with elderly animals.
-
8m 15s
Takenaka Yoko runs a local estate agency.
-
8m 20s
Aged around two and a half,
-
8m 22s
her pet poodle Kotaro was rescued from an abusive home.
-
8m 28s
He slips a lot because he can't plant his feet.
-
8m 34s
Kotaro is missing the lower portion of his right front leg,
-
8m 38s
while a snapped Achilles tendon in his left hind leg
-
8m 42s
means he is forced to get around on only two limbs.
-
8m 55s
It's OK, sweetheart.
-
9m 00s
Since Takenaka took him in six months ago,
-
9m 03s
they've yet to go for a single walk outside.
-
9m 08s
I tried to persuade him to come out with me, but he hates going out.
-
9m 14s
He's not used to it, so he gets scared,
-
9m 17s
and he won't walk on the pavement,
-
9m 19s
because it's too hard for him.
-
9m 22s
I hope that we can help him enjoy going out like a normal dog.
-
9m 33s
With Kotaro's regular veterinarian
-
9m 35s
deciding surgery would be of limited use,
-
9m 38s
they turn to Shimada for custom mobility aids.
-
9m 41s
And today, they'll be trying the completed equipment out for the first time.
-
9m 47s
70% of a dog's bodyweight is supported by the front legs,
-
9m 52s
so Shimada has prepared a prosthetic extension
-
9m 54s
to help Kotaro's right foot reach the ground.
-
10m 00s
For his left hind leg meanwhile,
-
10m 03s
Shimada has created a brace to compensate for the damaged Achilles.
-
10m 11s
Looking good.
-
10m 16s
But will Kotaro be able to walk?
-
10m 28s
Oh, is that tough, baby?
-
10m 31s
At first, he seems to be struggling.
-
10m 34s
I guess it's the extra length. He can't coordinate.
-
10m 41s
They decide on an extended trial
-
10m 43s
to see if Kotaro can get used to his new aids.
-
10m 50s
And a month later, how is he getting on?
-
11m 07s
He's taken to his new life with flying colors
-
11m 10s
and is now able to walk confidently on four legs, even outdoors.
-
11m 18s
Now he can go right up to people,
-
11m 21s
and he gets all this attention from everyone.
-
11m 26s
We're very happy.
-
11m 33s
Shimada Akio's passion for prosthetics
-
11m 36s
stems from direct family experience.
-
11m 41s
His grandfather Senkichi lost several fingers in an accident
-
11m 45s
at the factory where he worked.
-
11m 47s
He, too, used prosthetic fingers,
-
11m 50s
which showed Akio the importance of such devices.
-
11m 56s
He would go on to study prosthetic design at college,
-
11m 59s
before joining a leading manufacturer of artificial limbs for humans.
-
12m 04s
But what prompted the switch of focus to animals?
-
12m 11s
My colleague's dog had an accident in which it injured its back.
-
12m 17s
I made a support by cutting a hole in a floor cushion.
-
12m 22s
And I realized there was a niche for stuff like that.
-
12m 27s
No one else was doing it but I thought I could.
-
12m 32s
In 2007, at the age of 26,
-
12m 36s
he founded Japan's first specialist animal prosthetics business.
-
12m 40s
But at first, things were far from plain sailing.
-
12m 45s
We'd present our products,
-
12m 47s
but owners and veterinarians need concrete evidence
-
12m 50s
before they'll use something.
-
12m 54s
They weren't selling at all,
-
12m 56s
and pretty soon it seemed like we might go under.
-
13m 03s
But Shimada stuck at it,
-
13m 05s
giving his all to the few orders they did receive,
-
13m 08s
placing his emphasis on quality over cost.
-
13m 15s
Wife Noriko, now part of Shimada's team,
-
13m 18s
remembers how hard he used to work.
-
13m 25s
We'd be out together and suddenly he'd get called out to a pet clinic.
-
13m 32s
I'd have to wait three or four hours in a net cafe
-
13m 36s
while he went to take care of that.
-
13m 41s
But you seemed to enjoy it.
-
13m 43s
Did I?
-
13m 45s
Totally absorbed. I knew he wouldn't give up.
-
13m 54s
Fast forward to November 2022, and we join Shimada in Nagoya,
-
13m 59s
where he's giving a presentation to an audience of 40 animal doctors
-
14m 03s
at a conference on the latest veterinary trends.
-
14m 09s
Recently, we're seeing a lot of ACL injuries.
-
14m 14s
He takes his audience through case studies
-
14m 17s
on how this common condition in older, larger, dogs
-
14m 20s
can be successfully addressed.
-
14m 23s
We use a brace for the ACL to stabilize the joint
-
14m 26s
and prevent meniscus tears.
-
14m 29s
Nowadays, with the value of prosthetics and orthotics
-
14m 33s
widely accepted within the veterinary medical establishment,
-
14m 36s
Shimada gets invited to give several dozen such talks each year.
-
14m 44s
Today's event was organized by veterinary surgeon Asai Ryota.
-
14m 51s
All our training is focused on surgical or pharmaceutical interventions.
-
14m 57s
Few veterinarians learn anything about orthotics or prosthetics.
-
15m 02s
It's a whole new world to us.
-
15m 04s
Shimada really is one of a kind.
-
15m 09s
These days, Shimada fields almost 3,000 orders per year.
-
15m 14s
And in the 15 years since he launched his business
-
15m 17s
he's helped over 20 thousand animals.
-
15m 21s
And his care isn't limited to dogs.
-
15m 24s
This cat with perpetually dislocated hind legs
-
15m 27s
was another beneficiary of Shimada's braces.
-
15m 38s
There's one way to make sure you win the trust of veterinarians,
-
15m 44s
and that's by making pet owners happy.
-
15m 51s
Back at his workshop, Shimada is working on a new kind of brace.
-
15m 59s
They're for this penguin.
-
16m 02s
An aquarium in western Japan
-
16m 04s
has reached out for help with one of its penguins.
-
16m 13s
The public facility in Hamada, Shimane Prefecture,
-
16m 16s
learned about Shimada through media coverage of his work.
-
16m 27s
The patient is Gin,
-
16m 29s
a Northern rockhopper penguin in his mid-20s senior years.
-
16m 36s
You can tell he's not comfortable with his right leg.
-
16m 42s
Gin's reluctance to place his weight on his right leg
-
16m 46s
indicates that he is in some pain.
-
16m 52s
He's suffering from bumblefoot,
-
16m 54s
a condition in which the pain stems from inflammation of the footpad.
-
16m 59s
Thought to be caused in part by inactivity,
-
17m 02s
it's common among captive birds,
-
17m 05s
and if left untreated it can even prove fatal.
-
17m 12s
Supervisor Tanabe Kento feels that
-
17m 14s
an orthotic support would help Gin to walk more naturally,
-
17m 18s
promoting exercise that may help relieve his condition.
-
17m 25s
Surgical and medicinal interventions could be a major strain,
-
17m 31s
and even present certain risks.
-
17m 36s
I feel supports might be a stress-free way to improve his symptoms.
-
17m 42s
That's the appeal.
-
17m 48s
But creating mobility aids for an aquatic bird
-
17m 51s
is completely different to dogs and cats.
-
17m 57s
Dogs have four legs, but penguins walk on two, just like humans.
-
18m 06s
The material this time is the same thermoplastic resin
-
18m 10s
that drink bottles are made of.
-
18m 17s
The substance becomes malleable when heated,
-
18m 20s
enabling Shimada to sculpt it into the necessary shape
-
18m 23s
over casts of Gin's own flippers.
-
18m 37s
As it cools, it solidifies to create a flexible, slip-on boot.
-
18m 48s
And these are lined with the same shockproof gel we saw earlier.
-
18m 54s
After assembly and some adjustments to the boot design,
-
18m 59s
Gin the penguin's ankle supports are complete.
-
19m 08s
It's not like some scientific experiment,
-
19m 12s
but when trying new approaches,
-
19m 14s
you get some strange results.
-
19m 17s
Some good, some bad.
-
19m 21s
I'm 80% nervous, 20% excited.
-
19m 31s
The new supports arrive by courier at the aquarium in Shimane.
-
19m 40s
Hello.
-
19m 42s
And Shimada is on hand remotely to guide the initial fitting.
-
19m 48s
I'm a bit worried about Gin swimming with these on.
-
19m 53s
Will they be OK for him to wear while swimming?
-
19m 58s
Sure. Pop them into a bucket of water.
-
20m 04s
Oh, they float!
-
20m 06s
To facilitate easy retrieval if they come off while swimming,
-
20m 10s
the supports are designed to float.
-
20m 14s
It looks like these should be OK.
-
20m 20s
Next, it's time for Gin to try on his new supports.
-
20m 39s
He doesn't seem to mind.
-
20m 45s
Some dogs get a bit confused and startled
-
20m 47s
the first time they try these things on.
-
20m 56s
Wow, he's jumping pretty well.
-
21m 01s
He seems to have stopped putting all his weight on his better foot.
-
21m 08s
He seems to be walking comfortably, with better posture.
-
21m 13s
Oh, really?
-
21m 16s
Fantastic.
-
21m 18s
Another job well done.
-
21m 23s
And the unexpected assignments don't end there.
-
21m 28s
We join Shimada on a visit
-
21m 30s
to a veterinary clinic in Tokyo's Setagaya Ward.
-
21m 38s
A month or so ago, they received an unusual patient.
-
21m 42s
A wild tanuki who'd been hit by a car.
-
21m 52s
Also called racoon dogs,
-
21m 55s
tanuki are a species of canid common throughout East Asia.
-
22m 00s
These omnivorous, nocturnal mammals
-
22m 03s
are often encountered even in central Tokyo,
-
22m 06s
and nationwide, they are a frequent victim of traffic accidents.
-
22m 16s
This poor creature's injuries were so severe that
-
22m 19s
both its front legs have had to be amputated.
-
22m 26s
The surgeon in charge was Sasaki Sumire.
-
22m 32s
She's still young so we thought we might be able to save her.
-
22m 39s
Where will she go? She doesn't have an owner.
-
22m 43s
To a rehab center for wild animals.
-
22m 49s
But before this tanuki can be sent to a zoo or NPO-run rehab center,
-
22m 55s
she will have to be able to get around and eat for herself.
-
23m 02s
I wonder if a wild animal can get used to prosthetics.
-
23m 10s
Though he's never worked with a wild mammal before,
-
23m 14s
Shimada agrees to create a prosthetic device.
-
23m 24s
There's always a first time,
-
23m 26s
and that's the starting point for all my work.
-
23m 30s
And I always want to help animals in need.
-
23m 38s
Back at the workshop,
-
23m 39s
the first challenge is how to create a natural posture.
-
23m 43s
Shimada gets down to cutting and shaping blocks of urethane foam.
-
23m 49s
Restoring this tanuki's mobility using only her hind legs
-
23m 52s
will call upon all of Shimada's experience and expertise.
-
24m 00s
My aim was never to make money.
-
24m 05s
I'm just lucky to have found something I love doing.
-
24m 13s
Once glued together,
-
24m 15s
the foam is carefully shaped to support the tanuki's posture.
-
24m 27s
And instead of front legs,
-
24m 29s
he decides to install wheels designed for radio-controlled cars.
-
24m 39s
After a process of trial and error,
-
24m 41s
the injured tanuki's mobility aid is complete.
-
24m 48s
I have absolutely no idea if this will work.
-
24m 54s
I'd never even seen a tanuki up close before.
-
25m 01s
Will this be OK?
-
25m 05s
I hope it works out...
-
25m 11s
The following day, Shimada takes the new design for a trial fitting.
-
25m 28s
Two months after her accident,
-
25m 30s
the tanuki's eyes have already started to brighten up again.
-
25m 37s
In terms of fit and slack,
-
25m 41s
I've aimed for halfway between a dog and a cat.
-
25m 49s
But will this wild animal accept her new mobility aid?
-
26m 12s
Good job!
-
26m 14s
She's walking.
-
26m 20s
In just three minutes,
-
26m 22s
she is already able to stand up and propel herself forward.
-
26m 32s
She's getting the hang of it.
-
26m 37s
If she can't walk, then feeding is a problem.
-
26m 41s
But this restores all kinds of possibilities.
-
26m 45s
And walking will help to build her strength back up too.
-
26m 51s
It forced me to draw upon all my experience and explore something new,
-
26m 56s
so this was a very valuable assignment.
-
27m 02s
I love animals, so when I look in their eyes,
-
27m 05s
I want to help them through.
-
27m 11s
I think this is one more animal we've managed to help survive.
-
27m 19s
Come on. She's getting worse.
-
27m 27s
Another day brings another fitting
-
27m 30s
for a Yorkshire terrier with dislocation issues.
-
27m 35s
She can't straighten her legs,
-
27m 37s
so this support does that for her.
-
27m 43s
And moving forwards,
-
27m 44s
Shimada Akio will continue to give a new lease of life
-
27m 48s
to stricken animals and their owners.
